Job summary

Pittsburgh Mercy is seeking a PATH Clinical Case Manager to provide outreach and support to individuals experiencing homelessness across the community. The role helps both chronic and non-chronic clients access housing, behavioral health services, substance use treatment, and public benefits.

  • Valid Drivers' License
  • Ability to operate organizational vehicle/van
  • Act 33/34/73 clearances
  • Pre-employment Drug Test

  • This role helps both chronic and non-chronic homeless populations access and navigate Allegheny County's Continuum of Care services, including housing assistance, behavioral health services, substance use treatment, and public benefits programs.
  • The position involves conducting outreach in a variety of settings, including shelters, encampments, community locations, and other agency sites.
  • The Clinical Case Manager connects individuals with resources, advocates for their needs, and supports them in achieving greater stability and access to essential services.
  • Accurate and timely documentation is required in both the county's HMIS system and Pittsburgh Mercy's Avatar reporting system.

SCHEDULE:

(hourly/non-exempt)

37.5 hours per week primarily Monday-Friday 8 am-4pm with flexibility to meet consumer and/or program needs. Some nontraditional hours (evenings, weekends, and holidays to meet the needs of the program.)

Success in this role requires flexibility, strong organizational skills, and the ability to manage multiple priorities while responding to the changing needs of persons served throughout the day.
