Job summary

Qualifications
- Licensed Physical Therapist (PT) in IL with IDFPR licensure.
- BLS certification within 30 days as required by American Heart Association.
Responsibilities
- Designs an appropriate plan of care with goals, treatment, outcomes, and discharge plan.
- Performs examination and selects reliable methods for evaluation of the chief complaint and history.
- Educates patients, families, and other health care providers using effective teaching methods.
- Monitors care delivered by support personnel and provides regular feedback.
- Communicates with the medical team and documents according to standards.
- Acts as a clinical instructor when assigned by leadership and participates in team activities.
