Job summary

Carle Health is seeking a licensed Physical Therapist to plan and administer PT treatment plans aimed at alleviating pain, restoring function, and promoting overall fitness. The role models professional service delivery and serves as a clinical resource for staff and students.

The incumbent will design care plans, adjust treatment based on patient response, educate patients and families, and collaborate with the medical team to achieve outcomes across outpatient orthopedic settings.

Qualifications

• Licensed Physical Therapist (PT) in Illinois; Basic Life Support (BLS) within 30 days.
• Bachelor's Degree in Physical Therapy required.
• Experience in outpatient orthopedics.

Responsibilities

• Performs patient transfers with assistance as needed while using safe body mechanics.
• Designs and adjusts plans of care integrating goals, treatments, outcomes and discharge plans.
• Educates patients and families using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes of care.
• Performs examinations with reliable assessment methods relevant to chief complaint and history.
• Screens patients to determine the need for therapy services and supports with staff.
• Monitors care delivered by support personnel and provides feedback.
• Communicates with medical team and documents according to standards.
