Job summary

Carle Health is seeking a licensed Physical Therapist to plan and administer treatment plans that alleviate pain, restore function, and promote overall health. You will serve as a role model and clinical resource for staff and students, with opportunities to mentor and lead teams across outpatient settings.

Responsibilities include designing care plans, educating patients and families, and collaborating with the medical team to achieve optimal outcomes.

Qualifications

• Licenses: Licensed Physical Therapist (PT) - IDFPR; Basic Life Support (BLS) within 30 days - AHA

Responsibilities

• Designs an appropriate plan of care that integrates goals, treatment, outcomes, and discharge plan.
• Educates patients, family, caregivers and other health care providers using relevant teaching methods.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
• Performs examination, selecting reliable and valid examination methods relevant to the chief complaint, results of screening, and history.
• Monitors the care delivered by support personnel and provides regular feedback.
• Communicates with medical team and documents according to department and practice standards.
• Meets customer service and productivity standards.
• Acts as clinical instructor when assigned by team lead/manager/supervisor.
