Job summary

Carle Health is seeking a licensed Physical Therapist to develop and administer treatment plans for patients to alleviate pain, restore function, and promote overall health. This role acts as a clinical resource and contributes to patient care across an outpatient setting.

Responsibilities include designing care plans, educating patients and families, evaluating outcomes, supervising staff, and collaborating with the medical team.

Qualifications

• Licensed Physical Therapist (PT) in Illinois (IDFPR) or eligible for licensure.
• BLS certification within 30 days of hire (AHA).
• Bachelor’s degree in Physical Therapy (Required).

Responsibilities

• Demonstrates patient transfers with assistance as needed.
• Maintains safe body mechanics and respectful patient care.
• Designs and updates plan of care with goals, treatment, and discharge.
• Adapts care based on patient response and evidence.
• Educates patients, families, and other providers.
• Evaluates findings to determine PT diagnoses and outcomes.
• Performs examinations using validated methods.
• Screens patients to determine therapy needs.
• Monitors care provided by support staff and provides feedback.
• Communicates with the medical team.
• Documents care per standards.
• Meets productivity and customer service standards.
• Acts as clinical instructor when assigned.
• Participates in team and department activities.
• Performs reporting and other duties as assigned.
