Job summary

Fresenius Medical Care seeks a professional Registered Nurse for the Outpatient RN CAP 2 designation, an entry level step into the Clinical Advancement Program for new employees meeting CAP 2 criteria or advancing from CAP 1. You will provide and coordinate clinically competent care across assessment, planning, intervention, and evaluation for an assigned group of ESRD patients.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful completion of CPR BLS Certification.
• Must meet the practice requirements in the state in which he or she is employed.

Responsibilities

• Performs all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor or in collaboration with other Registered Nurses.
• Performs ongoing data collection and analysis of patient data pre – during – post hemodialysis treatment and documents in the medical record.
• Assesses and documents patient/family learning needs to provide education.
• Directs daily delivery of safe and effective hemodialysis therapy for assigned patients with the care team.
• Administers medications as prescribed and documents justification and effectiveness.
• Initiates or assists with emergency response measures.
• Acts as a resource, mentor and role model for new and incumbent staff at the facility and area level.
• Ensures correct laboratory collection, processing and shipping procedures are followed.
• Identifies outcomes and updates nursing assessments and care plans in collaboration with the Interdisciplinary Team.
• Provides patient education regarding transplant and treatment modality options.
• Participates in facility and area quality improvement projects.
