Job summary

Continuum Therapy Partners in Lake City, FL seeks a licensed Occupational Therapist for PRN daytime coverage at Lake Montgomery Health & Rehabilitation Center. You will work within a supportive team, evaluating and treating patients to improve functional independence.

Minimum requirements include a Master's or Doctorate in OT, active OT license in Florida, NBCOT certification, and prior OT experience is preferred. This flexible role offers autonomy and collaborative care.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy.
• Current state OT license and NBCOT certification.
• 1 year of OT experience preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.

Occupational Therapist Requirements & Responsibilities

• Master's or Doctorate Degree in Occupational Therapy required
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred
• Evaluates and develops individualized treatment plans to improve patients' functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
