Job summary

Continuum Therapy Partners is seeking an Occupational Therapist to provide PRN services at The Lilac at Bayview in St. Augustine, FL. The role supports a team of therapists who emphasize quality care and flexible scheduling to fit your life.

The candidate will evaluate and develop treatment plans, document patient progress, and educate patients and families on adaptive equipment and home safety. A master's or doctoral degree in OT and an active OT license with NBCOT certification are required.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
