Job summary

Continuum Therapy Partners, LLC is seeking a licensed Occupational Therapist for a PRN role in St. Augustine, FL. This position offers the freedom to choose when you work while remaining part of a supportive clinical team that prioritizes quality patient care.

You will evaluate patients, develop individualized plans, and educate families on home safety and adaptive equipment. Prior experience and NBCOT certification are preferred, with duties aligned to a collaborative, patient-centered model.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.

Occupational Therapist Requirements & Responsibilities

• Master's or Doctorate Degree in Occupational Therapy required
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred
• Evaluates and develops individualized treatment plans to improve patients’ functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
