Job summary

Continuum Therapy Partners, LLC in Sheboygan, WI is seeking a licensed Occupational Therapist for PRN scheduling. You will join a team of clinicians who collaborate to deliver patient-centered care while enjoying the flexibility to choose your hours.

Requirements include a Master's or Doctorate in Occupational Therapy, current state license, NBCOT certification, and a preference for at least one year of experience.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy is required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.
• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes; Educates patients and families on adaptive equipment and home safety recommendations.
