Job summary

Continuum Therapy Partners is seeking a licensed Occupational Therapist for a PRN daytime role at Odd Fellow Home in Green Bay, WI. You will apply your clinical voice to evaluate, plan, and implement care that supports patients' independence.

Requirements include a Master's or Doctorate in Occupational Therapy, current Wisconsin OT license with NBCOT certification, and at least 1 year of experience preferred. You will work with a supportive team and families in a collaborative setting.

Qualifications

• Evaluates and develops treatment plans to improve functional independence.
• Educates patients and families on safety and equipment.

Occupational Therapist Requirements & Responsibilities:

• Master's or Doctorate Degree in Occupational Therapy required

• Current, unencumbered state Occupational Therapy License; NBCOT Certification required

• 1 year of previous experience as an Occupational Therapist preferred

• Evaluates and develops individualized treatment plans to improve patients' functional independence

• Documents patient progress and modifies treatment plans based on clinical outcomes

• Educates patients and families on adaptive equipment and home safety recommendations
