Job summary

Continuum Therapy Partners is seeking an Occupational Therapist to join Cedarburg Health Services in Cedarburg, WI on a PRN basis. The role emphasizes flexible daytime scheduling and collaborative care with a seasoned therapy team.

Required are a Master’s or Doctorate in Occupational Therapy, current state license, and NBCOT certification, with a preference for 1 year of experience in OT to support patient independence and safety.

Qualifications

• Master's or Doctorate in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.
• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
