Job summary

Continuum Therapy Partners, LLC in Wisconsin is seeking an Occupational Therapist for PRN daytime coverage at Menomonee Falls Health Services.

• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
