Job summary

Continuum Therapy Partners is seeking a PRN Occupational Therapist for the Menomonee Falls Health Services location in Wisconsin. The role focuses on flexible daytime PRN work while maintaining team collaboration and high quality care. Ideal candidates have OT licensure, NBCOT certification, and at least 1 year of experience.

You will evaluate, plan, document progress, and educate patients on adaptive equipment and home safety as part of a patient-centered care approach.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.
• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
