Job summary

Continuum Therapy Partners is seeking an Occupational Therapist for a PRN, on-site role at Oakwood Health Services in Altoona, WI. You will work with a supportive team to deliver high-quality care and flexible scheduling.

The role requires a Master's or Doctorate in OT, valid OT license and NBCOT certification, and prior experience is preferred. You will evaluate patients, develop treatment plans, document progress, and educate families on equipment and safety.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state OT license; NBCOT certification required.
• 1 year of previous experience as an Occupational Therapist preferred.
• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
