Qualifications

• Licensed Physical Therapist (PT) in good standing.
• BLS certification within 30 days.
• Continuing education in care of acute stroke patients.

Responsibilities

• Performs patient transfers with safe body mechanics.
• Designs and adjusts a plan of care with goals and discharge plan.
• Educates patients, families, and staff on care strategies.
• Evaluates findings and determines PT diagnoses and outcomes.
• Documents in EMR and communicates progress with providers.
