Qualifications

• Bachelor's degree in Physical Therapy (PT) required.
• Licensed Physical Therapist (PT) in IL.
• Basic Life Support (BLS) within 30 days (AHA) required.
• Attend and satisfactorily complete required CE for acute stroke care.

Responsibilities

• Demonstrates safe and effective patient transfers with assistance as needed.
• Designs and adjusts plan of care based on patient response and evidence.
• Educates patients, families, and caregivers using effective teaching methods.
• Evaluates findings to determine PT diagnoses and outcomes of care.
• Performs examinations using reliable methods aligned to chief complaint and history.
• Documents in EMR in line with CTSCP67 policy.
• Screens patients to determine need for further therapy services.
• Monitors care delivered by support staff and provides feedback.
• Completes annual education for DNV Hip/Knee program when applicable.
• Evaluates impairments related to orthopedic/neurologic conditions and plans care.
