Job summary

Carle Health is seeking a Licensed Physical Therapist to create and modify orthotics and to evaluate and treat patients of all ages. You will design plans of care that integrate goals, treatment approaches, outcomes, and discharge planning.

This role involves educating patients and staff, documenting in the EMR, and collaborating with other providers. A sign-on bonus is offered: $5,000 for new grads and $7,500 for those with one year of experience; relocation assistance is available.

Qualifications

• Licensed Physical Therapist (PT) – Illinois license/IDFPR required
• Basic Life Support (BLS) certification within 30 days of hire (AHA)
• Attend and satisfactorily complete continuing education related to acute stroke care

Responsibilities

• Evaluates and treats patients of all ages, creating and modifying plans of care.
• Designs plans of care integrating goals, treatment, outcomes, and discharge planning.
• Educates patients, families, and staff; documents in EMR; communicates progress to referring providers
