Qualifications

Responsibilities

• Demonstrates safe body mechanics and assists with patient transfers.
• Designs a care plan aligning goals, treatment, outcomes and discharge planning.
• Educates patients and caregivers using effective teaching methods.
• Assesses clinical findings to determine PT diagnoses and care outcomes.
• Performs examinations using reliable methods relevant to complaints and history.
• Documents care according to policy and clinical workflows.
• Monitors therapy delivery and provides feedback to staff.
