Job summary

Community Health Care Inc. is seeking an Oral Health and Dental Care Coordinator to facilitate access to preventive dental services for pediatric and adult patients.

You will work with dental and primary care teams to identify care gaps, coordinate referrals, and promote timely dental visits, while supporting HRSA quality measures and care coordination across populations. Qualifications

• High school diploma or GED required.
• Completion of a recognized Dental Assistant or Medical Assistant training program preferred.
• Associate degree in healthcare, dental health, public health, human services, or related field preferred.
• Dental Assistant, Medical Assistant, Community Health Worker, or comparable certification preferred.

Responsibilities

• Identify pediatric and adult patients with outstanding preventive dental needs using EHR and registries.
• Coordinate referrals and scheduling for preventive dental services.
• Collaborate with dental and primary care teams to close care gaps.
• Proactively outreach to patients, caregivers, and guardians for services.
• Identify barriers to dental care and assist with navigation of benefits and resources.
• Maintain accurate documentation and track care-gap interventions.
