Primary Health Solutions seeks a Dental Navigator to coordinate school-based and mobile dental services, identify barriers to care, and streamline patient scheduling. The role serves as a liaison among students, families, schools, community partners, and dental providers to ensure continuous, accessible oral health services.

The navigator promotes oral health education, secures consents, coordinates mobile visits, and supports preventive and restorative care.

Responsabilidades

• Coordinates mobile dental unit schedules with schools, community organizations, and partner sites.
• Ensures school-based consents and registrations are complete, and tracks follow-up needs.
• Develops efficient patient flow processes to maximize provider productivity with minimal disruption to school activities.
• Maintains communication with school administrators regarding schedules and participation.
• Tracks performance metrics, patient volumes, and program outcomes.
• Coordinates dental screenings and preventive programs within assigned schools.
• Works with school nurses and administrators to identify students with unmet dental needs.
• Tracks and manages consent forms for school-based dental services.
• Ensures timely follow-up for students requiring additional dental treatment.
• Promotes positive relationships with staff, patients and providers.
• Maintains confidentiality of patient information and adheres to mission and values.
• Maintains databases, assumes responsibilities, and performs assigned duties.
