Open Door Community Health Centers is seeking a Dental Treatment Coordinator in Eureka, CA. Under the Office Manager, you will identify urgent-care patients per CAMBRA, track treatment progress, and manage referrals to hospital dentistry.

You will follow up on external referrals for active patients and help coordinate transportation and language support as needed. You will build and maintain a resource/referral system across dental sites, promote patient education, and collaborate with providers

Competenze

Strong interpersonal and communication skills essential for collaboration with diverse patients and staff.
Ability to document clearly, chart patient information, and coordinate with providers.

Mansioni

Identify and assess clinic patients for urgent care following CAMBRA guidelines and track completion of treatment plans.
Coordinate and track cases referred out for hospital dentistry by primary care providers.
Follow up on outside referrals for active patients and assist with scheduling and transportation needs as needed.
Develop and maintain a resource and referral system within dental clinics and establish relationships with community partners.
Promote effective patient education and facilitate communication between patients and dental providers.
Participate in chart reviews, case management activities, and QA processes as directed.
