• Conduct structured social needs assessments and connect patients with community resources (transportation, financial assistance, food, equipment, emotional support).
• Support development of patients' care plans and participate in care conferences for quality delivery.
• Serve as the primary point of contact for virtual visit readiness and basic technology troubleshooting.
• Coordinate and schedule care including follow-ups and referrals.
• Prepare structured charts to ensure providers are visit-ready with accurate contact, insurance and care team details.
• Track completion of symptom surveys and ensure timely follow-ups and escalations as needed.
• Document social interventions and care coordination activities in the EHR in a timely, standardized format.
