Daymark Health Inc. is seeking a Care Coordinator to support Nurse Practitioners and Registered Nurses with in-home and virtual visits, ensuring timely scheduling and patient follow-through. You will communicate warmly with patients and families, handle records requests, and help keep care moving smoothly.

The role emphasizes meticulous documentation, HIPAA-compliant processes, and collaboration across the Care Team to meet patient needs and deadlines. Onsite in Providence, RI.

Competenze

• 3+ years of experience in medical office administration, patient scheduling, care coordination, or similar healthcare setting.
• Experience in oncology, home health, or virtual care is a plus.
• Strong calendar management and communication skills are essential.

Mansioni

• Schedule and manage in-home and virtual visits for Nurse Practitioners and RNs, optimizing calendars and minimizing gaps.
• Coordinate with patients and caregivers to schedule, confirm, and reschedule visits, including missed appointments and follow-ups.
• Develop knowledge of your patient panel to prioritize visits and surface patient needs to providers.
• Provide coverage for inbound patient calls and assist with scheduling questions, transferring to appropriate team members.
• Process medical records requests in line with HIPAA and Daymark policies, send/receive faxes securely.
• Maintain high operational excellence with clear documentation in EHRs and adherence to policies.
• Collaborate across Care Team to cover urgent needs.
