Job summary

Daymark Health Inc. is seeking an Oncology Nurse Navigator to join a collaborative Care Team providing triage, case management, and care navigation for oncology patients. The role emphasizes virtual patient interaction, education, and coordination across clinicians and social supports.

This full-time, remote position runs Monday through Friday, 8:30 AM–5 PM ET. Preference may be given to RI-based candidates willing to visit the RI office weekly for meetings.

Qualifications

• Bachelor of Science in Nursing is required.
• Compact unrestricted RN license, RI license or eligibility.
• 3+ years nursing experience, 2+ years in oncology.
• Oncology certifications (OCN, AOCN, AOCNS, CCM) preferred or obtainable within 2 years.
• Comfort with Slack, Zoom, Google Suite, and EHR systems.
• Strong relational and communication skills; patient-centered.
• Bias to action; proactive in urgent needs.

Responsibilities

• Provide direct patient care through virtual platforms.
• Deliver high-quality care across oncology care journey from screening to survivorship or end-of-life care.
• Triage and respond to Symptom Survey and patient-reported symptoms.
• Coordinate care including nutrition, discharge planning, and referrals.
• Educate patients about diagnosis, Daymark Health offerings, and next steps.
• Build rapport with patients and care networks with empathy.
• Collaborate with nurses, medical directors, social workers, and health partners.
• Adhere to policies, procedures, and documentation standards.
