Daymark Health in Providence, RI is seeking a Care Coordinator to ensure timely visits with Nurse Practitioners and RNs, and to support patient communication and administrative tasks from home or video.

You will schedule, confirm, and adjust visits, provide basic tech support for virtual visits, and help maintain HIPAA-compliant records. A fast-paced, collaborative environment awaits with opportunities to contribute across the care team.

Competenze

• 3+ years of experience in medical office administration, patient scheduling, care coordination, patient access, or similar healthcare setting.
• Experience in oncology, home health, or virtual care model is a plus.
• Strong calendar management and prioritization abilities.
• Warm, clear communication skills with patients and families.
• Organized, detail-oriented with HIPAA familiarity and privacy.

Mansioni

• Schedule and manage in-home and virtual visits for nurse practitioners and registered nurses.
• Coordinate with patients to schedule, confirm, and reschedule visits; provide basic tech support for virtual visits.
• Develop knowledge of patient panel priorities and upcoming needs to predict visits.
• Provide coverage for inbound patient calls and route questions to the right team.
• Process medical records requests; send/receive faxes securely.
• Maintain documentation in EHR and adhere to policies and timelines.
• Collaborate across Care Team to cover urgent needs.
