Daymark Health Inc. is seeking a Care Coordinator to join our Daymark Care Team in Providence, RI.

You will schedule and manage in-home and virtual visits for nurse practitioners and registered nurses, coordinate with patients and caregivers, and provide basic tech support for virtual visits. This role emphasizes attention to detail, HIPAA-compliant documentation, and proactive outreach to ensure timely care.

Competenze

• 3+ years of experience in medical office administration, patient scheduling, or care coordination.
• Experience in oncology, home health, or virtual care is a plus.
• Calendar management skills to juggle multiple calendars and last-minute changes.
• Warm, clear communication with patients and families.

Mansioni

• Schedule and manage in-home and virtual visits for the Nurse Practitioners and Registered Nurses you support, optimizing calendars and minimizing gaps.
• Coordinate with patients and caregivers to schedule, confirm, and reschedule visits and follow-ups.
• Provide basic technical support to patients for virtual visits.
• Develop knowledge of your patient panel to prioritize care needs and surface issues to providers.
• Provide coverage for inbound patient calls and resolve scheduling questions; route to appropriate team member as needed.
• Process medical records requests in line with HIPAA and Daymark policies; handle faxes securely.
• Maintain high standards of documentation and adhere to policies; track deadlines and productivity metrics.
• Collaborate across Care Team to cover urgent needs and maintain smooth operations.
