Job summary

Daymark Health Inc. is seeking a Health Partner to coordinate social and logistical aspects of care for patients. You will connect with patients by phone, video, and in-home visits, help navigate community services, and ensure follow-up appointments are scheduled.

This full-time, onsite role requires strong prioritization, attention to detail, and the ability to deliver high-quality work quickly in a fast-paced environment. BLS certification and vaccination/testing compliance are required.

Qualifications

• 3+ years of experience in healthcare navigation or patient support.
• Comfort with Slack, Zoom, Google Suite, and EHR systems.
• Bias to action and ability to prioritize.
• Comfort with change and ambiguity in a dynamic environment.
• Strong relational skills with diverse populations.
• Highly organized and detail oriented, with clear communication.
• Open to working from Daymark's local office in Providence, RI five days a week, and visiting patients' homes.

Responsibilities

• Conduct structured social needs assessments (SDOH) and connect patients with community resources.
• Support development of patients’ care plans and participate in case conferences.
• Serve as the primary point of contact for virtual visit readiness and basic technology troubleshooting.
• Coordinate and schedule care for patients, including follow-up and referrals.
• Own structured chart preparation to ensure providers are visit-ready.
• Track completion of Symptom Survey and escalate clinical concerns as needed.
• Document social interventions and care coordination activities in the EHR.
