Qualifications

Responsibilities

• Provide clinical nursing care using the nursing process.
• Coordinate care with the interdisciplinary team and patient’s physician.
• Perform phone follow-up with patients and families as needed.
• Educate patients and families on hospice care within scope of practice.
• Document clearly per RC guidelines and maintain care plans.
