Job summary

Hospice of the Western Reserve (Reserve Care) seeks an On-Call Nurse to support patients and families in after-hours care. You will visit patients at home, assess needs, provide intermittent skilled nursing, and collaborate with physicians to manage pain and symptoms.

You will operate within a compassionate team, handle documentation, and participate in triage as needed. Ohio RN/LPN licensure and strong communication are required, with a Bachelor’s preferred.

Qualifications

• Graduate of an accredited nursing program; Ohio RN/LPN license required.
• Bachelor's degree in nursing preferred; strong communication skills.
• Ability to work independently and with a team in home-based hospice care.

Responsibilities

• Assess patient and family needs (RN/LPN as applicable).
• Provide intermittent skilled nursing services under physician direction.
• Monitor pain and symptoms; educate patient and family on management.
• Liaise with physicians and coordinate information with the care team.
• Provide emotional support and refer to additional services as appropriate.
• Document and maintain records; ensure confidentiality.
• Participate in team meetings and mandatory trainings.
• Assist in triage coverage as needed.
• Perform other duties as assigned.
