Job summary

Reserve Care in Lorain County, OH is seeking an on-site Registered Nurse (RN) Support Nurse-Onsite to provide compassionate hospice and palliative care. You will use the nursing process to support patients and families, and collaborate with an interdisciplinary team to plan and coordinate care that aligns with patient goals.

Responsibilities include after-hours phone follow-up, documentation per RC guidelines, and determining when referrals to hospice are appropriate, while maintaining high

Qualifications

• Maintains a current Ohio RN license.
• Minimum two years hospice/palliative care experience preferred.
• Strong written and oral communication skills.
• Knowledge of hospice clinical standards and RC policies.
• Ability to collaborate with RC employees, community, and healthcare providers.

Responsibilities

• Gathers information, assesses, plans, implements and evaluates patient care using the nursing process.
• Collaborates with the interdisciplinary team and patient’s primary physician on care plans.
• Provides phone follow-up and post-discharge support to patients and families.
• Coordinates care within the agency and with external partners.
• Documents conversations and outcomes following RC guidelines.
• Identifies when referrals to hospice level of care are needed.
