Reserve Care is seeking an On-Call Nurse in Lorain, OH. This part-time role involves after-hours visits in patients' homes and requires RN/LPN licensure in Ohio with strong clinical skills.

Qualifikationen

• High School Diploma or equivalent.
• Bachelor's degree in nursing preferred.
• Licensed RN/LPN in Ohio.
• Experience in pain and symptom management preferred.

Aufgaben

• Assess patient and family needs (RN/LPN)
• Provide intermittent skilled nursing services under physician direction
• Monitor and manage pain and symptoms
• Provide education to patient and family on care at home
• Maintain documentation and coordinate with the care team
• Communicate with physicians and team members regularly
• Participate in team meetings and trainings
• Perform other duties as assigned
