Reserve Care is seeking a dedicated Registered Nurse to facilitate hospice and palliative care, supporting patients and families with expert clinical practice and compassionate guidance.

The role emphasizes collaboration with interdisciplinary teams, effective communication, and adherence to RC standards in a dynamic care setting. A current Ohio RN license and hospice experience are preferred, with opportunities for professional development.

Qualifications

• Must hold a current Registered Nurse license in the state of Ohio.
• Minimum of two years’ hospice/palliative care experience preferred.
• Flexible, problem-solving skills; strong prioritization and coping abilities.
• Excellence in written and oral communication.

Responsibilities

• Provide patient-centered care using the nursing process across interdisciplinary teams.
• Communicate with primary physician and team about patient condition and plan of care.
• Perform timely phone follow-ups and coordinate with care solutions center.
• Support referral criteria and coordinate intra-agency and external partnerships.
• Document consistently according to RC guidelines and care plan.
