Job summary

Reserve Care is seeking an on-site Registered Nurse (RN) to support palliative and hospice patients in Lorain County, OH. The role involves direct patient care, family education, and coordination with interdisciplinary teams to optimize comfort and quality of life.

The ideal candidate holds an Ohio RN license, with at least two years in hospice/palliative care, and demonstrates strong communication, collaboration, and problem-solving skills.

Qualifications

• Maintains a current Registered Nurse license in the state of Ohio.
• Minimum of two years’ hospice/palliative care experience preferred.
• Flexible, problem solving, prioritization and coping strategies.
• Excellent written and oral communication skills required.
• Knowledge of hospice/palliative care standards and RC guidance.

Responsibilities

• Provide direct patient care using nursing process to meet physical and emotional needs of patients and families.
• Coordinate care with interdisciplinary team and primary physician.
• Perform phone follow up with patients and families as needed and document outcomes.
• Support referrals and ensure appropriate level of hospice care is maintained.
• Document consistently per RC guidelines and maintain patient care plan alignment with goals.
