Reserve Care is seeking a licensed RN to provide compassionate hospice and palliative care in the Cleveland area. You will assess patient and family needs, coordinate care plans with a transdisciplinary team, and support symptom management under physician direction.

Qualifications include Ohio RN license, CPR, and a driver’s license; hospice experience preferred and certification within 2 years. On-call and discharge planning are part of the role.

Qualifications

• Licensed RN in the State of Ohio.
• Current CPR certification.
• Valid driver’s license.
• Hospice care, home health, oncology, or pain/symptom management experience preferred.
• Hospice and Palliative Care Certification desired; must be obtained within first 2 years of hire.

Responsabilités

• Provide intermittent skilled nursing services under physician direction.
• Assess physical, psychosocial, and spiritual needs of patients and families.
• Coordinate Plan of Care with the transdisciplinary team.
• Monitor and manage pain and symptoms; support patient and family.
• Participate in on-call rotations and discharge planning.
• Provide health teaching to patients and families; complete plans/reports.
