Job summary

Reserve Care is seeking an experienced Registered Nurse (RN) to provide compassionate hospice and palliative care in collaboration with a transdisciplinary team. You will support patients and families, coordinate the Plan of Care, and participate in on-call rotations and discharge planning.

The role requires Ohio RN licensure, current CPR, and a valid driver's license. Hospice certification is preferred and may be pursued within two years of hire.

Qualifications

• Licensed RN in the State of Ohio.
• Current CPR certification.
• Must have and maintain a valid driver’s license and reliable auto insurance.
• Hospice certification is desirable and should be obtained within the first two years of hire.

Responsibilities

• Provide prescribed medical treatment and personal care to terminally ill patients as part of a transdisciplinary team.
• Assess patient and family needs across physical, psychosocial, and spiritual domains.
• Deliver intermittent skilled nursing services under physician direction.
• Monitor pain and symptoms and guide patient/family in management.
