Job summary

Reserve Care, formerly Hospice of the Western Reserve, is seeking a Registered Nurse (RN) to serve as a Support Nurse-Onsite in the office setting in Ashtabula, OH. The role focuses on palliative and hospice care, providing education, symptom management, and coordination with the care team to improve quality of life for patients and families.

Qualifications

• Active Ohio RN license.
• Experience in hospice/palliative care preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Gathers information, assesses and plans patient care using the nursing process.
• Maintains collaboration with the interdisciplinary team and patient’s primary physician.
• Provides phone follow-up and after-hours triage for patient concerns.
• Coordinates care with primary nurse, social worker and other team members.
• Documents care activities per Reserve Care guidelines.
