Job summary

Lenawee Community Mental Health Authority in Adrian, Michigan is seeking a full-time, onsite Case Manager to support adults diagnosed with intellectual and developmental disabilities (I/DD).

The starting salary is $53,976 with an annual range of $53,976 to $69,810, based on years of service and a 37.5-hour work week. This role emphasizes coordination with community partners to foster resilience and recovery.

Qualifications

• Bachelor's degree in a human services field from an accredited program.
• Licensed by the State of Michigan to practice in a human services field (if applicable).
• Education, training and/or work experience in the behavioral health field with individuals with developmental disabilities.
• Basic computer knowledge (email and internet use) and experience with Microsoft Office applications (Word, Excel, Power Point, etc.).
• Valid driver's license.

Responsibilities

• Help individuals with developmental disabilities navigate systems to access supports.
• Develop a strength-based plan and a service plan tailored to consumer needs.
• Link and coordinate services and inter-agency resources.
• Monitor progress and facilitate transition strategies.
