Job summary

Riverwood Center in Southwest Michigan is seeking a dedicated behavioral health case manager to serve individuals with intellectual and developmental disabilities and mental health needs in the community. You will assess needs, develop person-centered plans, and coordinate access to health, education, employment, and social supports with families and providers.

A Bachelor's in Social Work or a Master's in a related field is required; QIDP experience and reliable transportation for home visits

Qualifications

• Bachelor in Social Work or Master’s in an appropriate mental health discipline.
• QIDP certification or equivalent preferred.
• Reliable transportation for home visits.

Responsibilities

• Provides community-based case management to individuals with intellectual and developmental disabilities and families.
• Facilitates linking, coordinating, and monitoring services to access health, financial, employment, education, social services and supports.
• Assesses needs, develops and monitors Person Centered Plans and documents treatment.
