As a Case Manager, you assist individuals with developmental disabilities in navigating systems to better meet their needs. You understand that traditional services often aren't effective for this population and look for opportunities to increase access to additional supports. You assist consumers in identifying supports, building a team and designing a strength-based plan that focusses on recovery.
