Job summary

Lenawee Community Mental Health Authority in Adrian, Michigan seeks a full-time, onsite Case Manager for adults with intellectual and developmental disabilities. The starting salary is $53,976 with a range up to $69,810 based on years of service.

Qualifications

• Bachelor's degree in a human services field
• Licensed by the State of Michigan to practice in a human services field (if applicable)
• Education, training and/or work experience in the behavioral health field with individuals with developmental disabilities
• Basic computer knowledge and experience with Microsoft Office applications (Word, Excel, Power Point, etc.)
• Valid driver's license

Responsibilities

• Assist individuals with developmental disabilities in navigating systems to meet their needs.
• Identify supports and build a team to design a strength-based plan focused on recovery.
• Coordinate services and inter-agency resources to close gaps in care.
• Monitor progress toward outcomes and develop transition strategies.
