Job summary

Cortica is seeking a Site Care Coordinator to join our growing multi-disciplinary team in San Rafael. This on-site role focuses on optimizing patient and provider scheduling to improve care delivery and operational efficiency, while serving as a primary contact for families.

You will collaborate with site leadership, clinical teams, and shared services to coordinate care, address scheduling concerns, and ensure timely case assignments within a patient-centered care model.

Qualifications

• 4+ years of scheduling experience in ABA services or related field.
• 2+ years in a client services related role.
• Strong problem-solving skills.

Responsibilities

• Guide patients through care pathways, monitoring progress and addressing delays.
• Be the on-site contact for families, streamlining communication with Cortica teams.
• Create scheduling plans to maximize provider capacity and meet client needs.
• Collaborate with clinical teams to integrate scheduling strategies into care plans.
• Provide scheduling support for schools contracts and Value-Based Care clients.
• Coordinate with on-site leadership to resolve scheduling concerns promptly.
• Use scheduling tools to improve efficiency and monitor KPIs.
