Job summary

ERC Pathlight is seeking an Alumni and Family Coordinator to support alumni, families, and patients across their region, based onsite at an integrated care facility. You’ll partner with clinical, outreach, and operations teams to coordinate aftercare planning, family programming, and connections to community resources, ensuring services are integrated into the patient experience.

This role requires 2+ years in a clinical or project-management setting, excellent communication, and strong

2+ years in clinical role or 2+ years in project management.
Strong organizational and project management skills.
Excellent professional verbal and written communication.

Responsibilities

Provides consultation in patient transition and discharge planning; coordinates referrals.
Leads onsite aftercare and discharge planning groups; connects patients to community resources.
Serves as subject matter expert across the patient journey from admission to discharge.
Coordinates onsite family programming and events.
Facilitates weekly virtual alumni and family support groups; follows up on referrals for higher levels of care.
Facilitates tours for prospective patients and families.
Coordinates alumni engagements and gathered testimonials from patients and families.
