Job summary

Qualifications

• Licensed pharmacist registered in Illinois with the IDFPR.
• Bachelor’s degree in Pharmacy is required.
• Clinical work experience in a hospital/clinical setting preferred.

Responsibilities

• Direct patient care to optimize prescribing, administration and monitoring.
• Provide MTM services and patient education.
• Make dosing recommendations based on age, weight and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams.
• Assist with clinical trials and investigational medications.
• Oversee preparation and dispensing of chemotherapy.
• Support transitions of care and participate in therapy planning.
