Qualifications

• Licensure as Pharmacist in Illinois (IDFPR).

Responsibilities

• Direct patient care to improve prescribing, administration, and monitoring practices.
• Provide MTM services and patient education as defined by state law.
• Determine optimal dosing based on age, weight, and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams for inpatient and outpatient care.
• Prepare investigational and standard medications for clinical trials.
• Attend rounds and provide pharmacy services in interdisciplinary care.
• Oversee chemotherapy preparation and transitions of care.
• Develop medication policies and participate in committees.
