Job summary

Carle Health seeks a Pharmacist on the oncology team to deliver comprehensive pharmaceutical care to patients and act as a clinical expert across departments. You will support hematology/oncology, urology, ophthalmology, and research initiatives with MTM, dosing guidance, and safe chemotherapy preparation.

Responsibilities include direct patient care, clinical rounding, and policy development, while supervising technicians and contributing to education for residents and staff.

Qualifications

• Licensed Pharmacist in Illinois (IDFPR) or eligible to obtain licensure in the state.
• Pharmacy degree from an accredited program.
• Clinical work experience in a hospital/oncology setting.

Responsibilities

• Direct patient care to optimize prescribing, administration, and monitoring.
• Provide MTM services and patient education per pharmacy practice act.
• Determine dosing based on patient age, weight, and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams.
• Prepare investigational and standard of care medications for trials.
• Attend rounds and provide oncology pharmacy services in the hospital.
• Ensure safe drug therapy management and monitor adverse effects.
• Oversee chemotherapy preparation and transitions of care.
• Support committees and participate in policy/protocol development.
