Job summary

Volunteers of America - LA is seeking an Entry-level Case Manager to provide front-line case management for clients in interim and permanent housing. The role involves assessment, care planning, coaching, care coordination and system navigation to support individuals who were experiencing homelessness and are now housed.

The position emphasizes on-site work, collaboration with a team, outreach, advocacy, and connection to direct services.

Qualifications

• Must pass fingerprint clearance and background checks (criminal history, references, employment and education verifications).
• Ability to work under emotional stress with at-risk populations.
• On-site work required; daily on-site presence needed.

Responsibilities

• Assess participant strengths and needs as part of a team.
• Provide coaching and social support to participants.
• Coordinate care and case management, including housing navigation and connections to services.
• Advocate for participants with other agencies and settings.
• Engage and connect participants to appropriate programs and resources.
• Outreach to locate participants and ensure program engagement.
