Job summary

Volunteers of America Los Angeles is seeking a Case Manager Generalist to coordinate services on-site for clients with low acuity, handling a diverse caseload and linking them to essential resources. The role emphasizes collaboration with a support team and community agencies.

You will conduct screenings, develop action plans, and advocate for clients to meet housing, health, and social service needs while ensuring safety and confidentiality.

Qualifications

• Fingerprint clearance/background check required.
• Ability to perform under emotional stress and with difficult clients.

Responsibilities

• Conducts comprehensive screenings and assessments to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information.
• Assess barriers and develop a case plan.
• Coordinate individualized planning with clients to meet short- and long-term needs.
• Provide service linkages and support systems to ensure identified needs are met.
• Coordinates case conferences with residents, consults with service team, and monitors progress on the case plan.
• Advocate for participants before agencies and communities as needed.

Education

HS/GED

Bachelor’s Degree in Social Services related field
