Entry-Level Case Manager — On-Site Housing Support

Job summary

VOALA is seeking an entry-level Case Manager to provide responsive case management for clients in interim and permanent housing. You will assess needs, develop care plans, coach social support, coordinate services, and help navigate the health and social service systems while engaging clients to access direct services.

On-site duties include assessing and coordinating plans with the team, advocating for clients, and connecting them to resources.

Qualifications

• Case management training within 6 months of hire.
• Associate’s Degree in Social Services related field (in lieu of experience)

Responsibilities

• Assess participant strengths and needs as part of Case Management team.
• Coaching and social support for participants.
• Coordinate care and case management and housing navigation.
• Advocacy for participants before other agencies and community settings.
• Capacity building—motivate participants and connect to programs.
• Outreach and engagement to bring participants into programs.

Education
