Job summary

Volunteers of America is seeking an Entry Level Case Manager to provide front-line case management for clients housed in interim and permanent housing. You will work as part of a team handling assessment, care planning, coaching, and connections to direct services.

This on-site role requires fingerprint clearance, background checks, and a California driver’s license. Education includes case management training within 6 months of hire; associate’s degree in social services is acceptable in lieu

Qualifications

• Fingerprint clearance and background checks required.
• Ability to handle emotionally challenging situations and difficult clients.
• On-site work requirement; report to site daily.
• California driver’s license preferred with clean driving record.

Responsibilities

• Assessment with the case management team to plan actions.
• Coaching and social support for participants.
• Coordinate care and case management with community services.
• Advocacy for participants before agencies and community settings.
• Capacity building and outreach to engage participants in programs.
• Other duties as required and may change.

Education
